Let your interview subjects talk with interrupting. If they are uncomfortable, have them talk more about their background before going on to their experience with death.

Print out the following questions and take them along to the interview.

THE QUESTIONS

Please tell me your full name and something about yourself. This answer should include home town, occupation, age, family make up. If it doesn’t, ask them those specifics.

Tell me about the day leading up to your death. Be sure they include their age at the time.

Cast you mind back to your death and describe the what happened in as much detail as you can.

What happened next? Repeat as necessary.

To Those Who Have Had A Near Death Experience 

.Have you had an experience where you were close to death, left your body, and traveled to another place? Was this place very real to you? Did this experience change you in any way? Chameleon Productions would like to film an interview with you for possible inclusion in

"Changing Planes," a documentary on what lies beyond death.

Who will do the filming?

1. You may know a videographer that you would like to have film your interview.

    Ask them to contact Chameleon Productions for information, instructions, and interview                   questions.

2. Chameleon Productions can help you locate a videographer/filmmaker in your area.

In addition to the interview, with your permission, the filmmaker would like to include additional footage like photographs of you at the age you were when you had the experience, any kind of supporting documetation you might have, perhaps views  of your home and the surroundings, perhaps a few words from someone who knew you and was around when you had the experience. You may have some ideas of your own to add.

Release Forms:

You and anyone else who appears in your interview footage will be asked to fill out a talent release at the time of your interview. Without this release Chameleon Productions cannot use your interview

NOTE:  Subjects of a documentary are traditionally not paid . This is to prevent the conscious or unconscious influence on the subjects to provide the answers the documentarian wants.  Only the absolute truth is acceptable.
